Dry Eye Treatments at Short Hills Ophthalmology

Dry eye caused by meibomian gland dysfunction does not always respond to drops alone. When the oil-producing glands in your eyelids become blocked or inflamed, the tear film breaks down faster than it should. Our practice offers several in-office treatments that target the underlying gland dysfunction rather than temporarily masking symptoms.

Intense Pulsed Light (IPL) for Dry Eye

IPL sends controlled pulses of broad-spectrum light to the skin around the eyes. The light energy targets abnormal blood vessels and inflammatory markers along the eyelid margin that contribute to meibomian gland blockage.

How IPL Works

One of the primary causes of evaporative dry eye is meibomian gland dysfunction (MGD). The meibomian glands produce the oil layer that prevents your tear film from evaporating too quickly. When those glands become blocked, the oil cannot reach the tear surface.

IPL addresses this by lowering inflammation around the glands, reducing bacterial load on the eyelid margin, and stimulating the glands to resume normal oil production. As gland function improves, tear quality stabilizes and dry eye symptoms decrease.

Cosmetic Benefit

Because IPL targets inflammation and abnormal vasculature, many patients also notice improvement in skin discoloration, redness, and fine lines around the treatment area. The same light energy that treats gland dysfunction has been used in dermatology for years.

OptiLight by Lumenis

OptiLight is the first and only IPL device with FDA approval specifically for treating dry eye caused by meibomian gland dysfunction. Developed by Lumenis, the same company that pioneered original IPL treatments, OptiLight uses proprietary Optimal Pulse Technology calibrated for the periocular area.

What OptiLight Targets

  • Inflammation along the eyelid margin
  • Bacteria and abnormal blood vessels feeding the inflammatory cycle
  • Rapid tear evaporation from compromised oil production
  • Reduced meibomian gland output

What to Expect During Treatment

Each OptiLight session takes about 15 minutes. Most patients feel a warm sensation from the gentle pulses of light. There is no recovery period needed afterward, and you can return to your normal activities right away. Many patients begin noticing improvement after 2 to 3 sessions, with results that build over the course of treatment.

LipiFlow Thermal Pulsation

Tear Film Layers and MGD

Your tear film has three layers: a mucin layer that holds tears against the eye surface, a watery aqueous layer that provides moisture, and an outer oil (lipid) layer produced by the meibomian glands. That oil layer prevents the watery tears from evaporating between blinks.

When meibomian glands become blocked, this protective oil cannot reach the tear surface. Tears drip or evaporate, causing burning, tearing, grittiness, and over time, potential corneal damage. Without the oil barrier, everyday activities like computer use, reading, and air-conditioned environments aggravate symptoms significantly.

How LipiFlow Works

LipiFlow is an in-office treatment that removes blockages from the meibomian glands using a combination of controlled heat and gentle pulsation. The system applies warmth to the inner eyelid surface while simultaneously massaging the outer lid, clearing hardened oil deposits from the glands.

The treatment takes approximately 12 minutes. Single-use sterile activators are contoured to avoid contact with the cornea, protecting the eye's delicate structures throughout the procedure. LipiFlow is supported by more than 30 patents and published studies demonstrating its safety and effectiveness.

LipiFlow Activators are single-use sterile devices that deliver a combination of heat to the inner lids and simultaneous therapeutic motion to the outer lids, removing blockages from the meibomian glands. The activators are contoured to avoid contact with the cornea, protecting the delicate structures of the eye during the procedure.

After Treatment

Once gland blockages are cleared, the glands can resume normal oil production. Maximum results typically appear 6 to 8 weeks after the procedure as gland function recovers. To maintain results for as long as possible, we recommend lid hygiene wipes, warm compress masks, omega-3 supplements, and regular blinking exercises.

TearCare for Evaporative Dry Eye

Localized Heat with Natural Blink

The TearCare System applies localized heat therapy to the eyelids using flexible SmartLid devices that conform to the natural shape of the lid. Unlike some other thermal treatments, TearCare allows you to blink normally during the warming phase, which helps your lids maintain their natural position and comfort.

After the heat cycle softens the blocked oil, your doctor performs manual expression of the meibomian glands to clear the remaining deposits. TearCare is designed specifically for adult patients with evaporative dry eye disease caused by MGD.

SmartLid Design

The flexible SmartLid design conforms to individual lid anatomy, and the open-eye approach allows for a more natural treatment experience compared to devices that require eye closure. Each SmartLid is single-use and adheres gently to the outer eyelid during the warming cycle.

BlephEx Lid Debridement

Removing Biofilm and Debris

For patients with blepharitis, bacteria and biofilm accumulate along the lash line and eyelid margins, triggering chronic inflammation that contributes to dry eye symptoms. BlephEx is a painless in-office procedure that uses a medical-grade micro-sponge to clean the edges of the lashes and eyelids, removing the bacterial biofilm and debris that drive the inflammatory cycle.

By clearing this buildup, BlephEx helps prevent inflammation of the tear glands and reduces the progression of chronic dry eye. The procedure is quick, typically taking about 6 to 8 minutes per eye, and most patients report immediate improvement in lid comfort.
